A paper copy of the Portal Activation will be given to you at the time of registration or a letter will be mailed to you. Your Primary Care or Specialty provider’s office will start the process of enrolling you in the patient portal. If you do not receive a code by April 5th, please contact us to request your activation code by calling our MEDENT Support Team at (716) 422-5801. If you are an active portal member, you will receive an email with directions to log in to the NEW portal.Īll your existing records will be securely transferred to the new patient portal. Your existing portal account is no longer active.
